Our office is closed on weekends and major holidays.
Appointments: We see patients by appointment
only.
We see patients who have a diagnosed heart problem and who are referred
to us by another doctor. If you are coming for your first office visit,
our scheduling staff will mail informational forms to you for completion
prior to your appointment. You may also download these forms from our

We ask that you bring these completed forms with you
to your appointment. In addition, please bring the following items with
you:
- All the medications that you are currently taking.
- Your insurance card and any referral forms from
your referring physician. (See the link for Insurance and Billing for
further details and a list of insurance plans with whom we participate).
- A recent chest x-ray and recent EKG.
- A copy of your medical records from your family
physician or the physician who referred you to our office.
Your First Office Visit:
Your initial appointment with one of our cardiac specialists will include
a discussion of your medical history, a physical examination, and if indicated,
any necessary diagnostic testing. We will evaluate whether your symptoms
are cardiac related and whether medical therapy and/or an interventional
procedure are necessary. If additional non-cardiac
evaluation is needed, we will be happy to recommend appropriate specialists
and coordinate your care with them. We do not take the place of your primary
care physician who will continue to provide for your general medical care.
A new patient visit usually requires approximately 45
minutes to one hour. We know your time is valuable and we try to stay
on schedule. We offer all of our patients the same thorough evaluation
on their initial office visit.
Please try to come about 15 minutes early for your first
appointment to complete any additional paperwork that we may need and
to allow time for the receptionist to get your complete record together
prior to your examination.
Return Visits:
If you are coming for a follow-up visit, be sure to notify our receptionist
of any address, phone number or insurance changes. Subsequent visits usually
take approximately 15 minutes.
Cancellations:
If you have made an appointment, but are not able to come, please call
us as far in advance as possible so we can use this time to treat another
patient. We will be happy to re-schedule your appointment at a more convenient
time for you.
Prescription Refills:
Requests for medication refills must be phoned in during regular office
hours. Call us at 859-277-7433 between 8:30 AM and 4:30 PM to refill your
prescription medications. Leave a detailed message and a nurse will return
your call to verify your refill request. Please have your pharmacy number
as well as your current prescription bottle available for reference.
Please do not request medication refills after hours,
on weekends or holidays, as the doctor on-call will not have access to
your medical records. Check to see how much medication you have on hand
so that you will not run out when the office is closed.
During office hours:
If you have a question or specific problem that requires a physician’s
expertise, please call us at (859) 277-5887 and we will notify your physician
and he will return your call.
After Hours/Weekends:
If you have an urgent problem, call us at (859) 277-5887, to connect you
to our answering service. The answering service will page the physician
on-call and notify him of your message. The on-call doctor will respond
to your request.
Emergencies:
If you think you have an emergency situation or think you are having a
heart attack, call 911.
Pro-Time Checks:
Medical Records:
Your medical records are absolutely confidential. You, the patient, are
the only one who can authorize any disclosure of your records. If you
want your records released, we will need you to sign a release authorization
form that instructs us how to handle your records according to your wishes.
The appropriate release forms are available from our receptionist.
Interpreter Services:
If English is not your primary language or if you are hearing-impaired,
an interpreter can be provided for you. Please notify our office schedulers
of any special needs prior to your scheduled appointment, so that we have
time in advance to arrange these services for you.
